Recommendation Request Form

This form allows applicants to request online recommendations for admission. Each person from whom you request a recommendation will receive an e-mail with a secure link to an online recommendation form. He or she can only submit an online recommendation by following the link in this e-mail.

Master of Divinity, Master of Arts, Master of Sacred Music and Certificate applicants must receive recommendations from at least three sources: one from a Pastor, one from a Teacher, and one from another who is not a family member. Recommendations can be received online, or on paper. Printable recommendation forms can be downloaded, and should be sent to the person writing the recommendation along with a stamped envelope addressed to Luther Seminary.

International Students and Graduate Students (M.Th., D.Min., Ph.D.) should not use this form. Online recommendations are not accepted at this time for these students.  However, recommendation forms can be downloaded and printed for International Students and Graduate Students.

Please do not use this form until you have submitted an application for admission, either online or by mail.
