Employment Forms

  • Form W-4
    Complete Form W-4 so that Luther Seminary can withhold the correct federal income tax from your pay.

  • Form I-9
    Student Employees need to complete the W-4 and I-9 to ensure that you will be paid while working on campus.

  • Direct Deposit Authorization
    Fill out this form to have your paycheck automatically deposited in you checking or savings account on payday.
