There are various form that you may need to fill out for events or special occasions on the Luther Seminary campus.

Request Space Form
Use this form to request a particular room, date and time for your meeting.

Media Equipment Request Form
Use this form to reserve media equipment, projectors, PA, phones, etc. This is for Luther faculty, staff, and students only. Media equipment is not rented to outside organizations.

Meeting Refreshment Order Form
Use this form to order refreshments for an upcoming meeting. If you have any questions about this form, please e-mail Dining Services.
