Due within one month of the beginning of the field experience. A copy must also be sent to your candidacy committee.<\/li>\n<\/ul>\n<ul>\n<li><a href=\"https:\/\/drive.google.com\/open?id=1d6VCv3AwEH7KpihxzCzRXf38YtFdIOzw\">Field Experience Student&#8217;s Evaluation<br \/>\n<\/a>One form from the student, following the completion of all field work. Send it in to the Contextual Learning office. A copy must also be submitted to your Candidacy Committee.<\/li>\n<\/ul>\n<h3>For supervisors\/project directors<\/h3>\n<ul>\n<li><a href=\"https:\/\/drive.google.com\/open?id=1gzcOhiHqyMwI-L1Hd6SWRRXZYRD7vAky\">Supervisor, Project Director Application<br \/>\n<\/a>One form from each person serving as either a field work supervisor or a project director. This must be on file in the Contextual Learning office.<\/li>\n<\/ul>\n<ul>\n<li><a href=\"https:\/\/drive.google.com\/open?id=1KDOiT6PnsWbgs_vQFSjeqoVh74WQCvmB\">Learning Agreement for Field Experience<br \/>\n<\/a>The candidate and Project Director, together in consultation, complete one form.
